Registration and Vaccination Requirements

The CAPCSD Business Meeting will be held in conjunction with the 2022 ASHA Convention and will require all attendees to adhere to state, city, hotel, and the ASHA Convention health requirements in place on November 16th.

 

All able and eligible participants attending the CAPCSD Business Meeting are required to be fully vaccinated (two-doses of a two-dose vaccine series or one-dose of a one-dose vaccination series). Unvaccinated individuals must refer to ASHAs Vaccination and Masking Policy for instructions on where to get an on-site PCR test to attend the CAPCSD Business Meeting.
 

Steps to Verify COVID-19 Vaccination

If you already have your vaccination verification through CrowdPass, skip this step.  You will be asked to upload  your vaccination verification when you register for the CAPCSD Business Meeting using the "Register Here!" link below.

 

If you need to get your vaccination verification, follow these steps:

  • Make a copy of your COVID-19 vaccination card. You will need to upload a copy in CrowdPass.
  • Complete the CrowdPass verification form. CrowdPass follows a two-step verification process that may take up to 12 hours. Most verifications are completed within an hour or two, but please be patient if you do not receive your confirmation until the next day.
  • When you receive your CrowdPass approval email, make a copy to upload with your CAPCSD Business meeting registration.
